North Shore-LIJ Health System Teams with Glenhaven Health Care Organization to Provide Comprehensive Rehabilitation Services on Long Island
GREAT NECK, NY - To meet the growing demand of patients requiring varied levels of rehabilitation after orthopedic surgery, the North Shore-Long Island Jewish (LIJ) Health System and the Glenhaven Health Care Organization have signed a clinical agreement of affiliation to provide a comprehensive program for the delivery of care to patients needing rehabilitation services.
The Glenhaven Health Care Organization, with its Glengariff Health Care Center in Glen Cove and Port Jefferson Health Care Facility, has provided sub-acute rehabilitation services and long-term care for patients at health system hospitals for more than 30 years. With the new agreement in place, North Shore-LIJ physicians will be able to streamline referrals and better monitor patients through their recovery process, offering patients optimum treatment services.
"The new agreement will enable us to offer a full continuum of care to patients who need rehab services," said Michael J. Dowling, president and chief executive officer of the 18-hospital North Shore-LIJ Health System. "The formal affiliation will ensure that health system quality mechanisms and standards are in place across the board. The goal is to provide seamless service to patients moving between facilities. Surgeons can be confident that whether a patient is in an acute rehab setting or a sub-acute rehab facility, rehabilitation staff will follow the surgeons' protocols. In turn, patients will be confident knowing that they are receiving the highest quality of care."
After orthopedic surgery, such as hip or knee replacement, a patient may require an in-hospital stay for acute rehabilitation. Acute rehab offers short-term, intensive physical and occupational therapy along with additional ancillary services with the goal of helping patients through the initial phase of recovery. Sub-acute rehab offers the same services through a modified rehabilitative program, which allows a patient a broad range of time to recover and return to an independent state of living. Some patients may also receive outpatient physical therapy after discharge.
"We are proud to be affiliated with the North Shore-LIJ Health System and to be recognized for our excellence in rehabilitation services," said Michael Miness, president of Glenhaven Health Care Organization. "For patients no longer needing acute rehabilitation, our facilities serve as an extension of the hospital. As patients move through recovery, we monitor them closely and help them build the skills needed for independent living in the community." Mr. Miness said that the average length of stay is under 25 days for patients in sub-acute rehabilitation.
Currently, the bulk of referrals from North Shore-LIJ to Glenhaven come from the Orthopedic Rehabilitation Institute (ORI) at North Shore University Hospital at Glen Cove, in addition to North Shore University Hospital in Manhasset and LIJ Medical Center in New Hyde Park. Other health system hospitals with orthopedic surgical programs will also refer patients to Glenhaven facilities for sub-acute rehabilitation as needed.
"The new affiliation agreement will further strengthen our relationship with the Glenhaven Health Care Organization," said Mark Stenzler, executive director of North Shore University Hospital at Glen Cove. "Now we will have the ability to provide the best continuity of care, while providing the highest level of outcomes for patients."
Officials said that the affiliation also provides the opportunity to explore the potential for developing joint clinical research studies as well as case management programs to improve the coordination of healthcare services. With the shared focus, there is potential for specialized health education training for staff, as well as patients and their families.
