Nursing facility sub-acute programs offer a diversity of medical, rehabilitative, and therapeutic services at a level of care between acute care and traditional nursing home care. Conditions treated in short-term rehabilitative sub-acute programs may include brain and spinal cord injuries, amputations, total hip and knee replacements, and cerebral vascular accidents. Sub-acute programs use an interdisciplinary team approach to patient assessment and treatment. Patients are typically discharged home after lengths of stay that, on average, range from a few weeks to three months.
